services such as routine check-ups, fillings, crowns, and dental surgery.

This plan is designed to work with the dentists the Provider has contracted with to
ensure the best rates, therefore if you visit a dentist outside of the contracted list,
you stand to pay a higher rate.

Here is a sample of the rates that Provider would pay.  You would pay the remaining
bill (assuming your deductible has been met).
